What happened
Associate was injured while performing a non-routine maintenance on a dock plate at dock door 16 in Zone 6. Associate was unjamming a dock plate that would not go to the ground-level position after a trailer had departed. The Associate went outside and untangled the mechanism chain under the dock plate resulting in a quick and unexpected closing of the plate to the ground shearing half of the tip of the left middle finger. Dock plate fell on associate 's fingers because the cylinder had failed and the chain was caught on underneath components and when the chain was released the Dock plate fell. Associate said that this was a simple task that happens infrequently. Associate was not aware of the back-up support mechanism under every dock plate that should be in place during any plate work.
