What happened

Fellow driver closed the driver 's trailer and pulled the dock plate in error. Driver opened door and failed to see that dock plate had been pulled. The pallet jack lunged forward in the gap between the trailer and the dock. He hit his chest on the pallet jack as it leaned forward into the gap.
