was asked to try to get an IV on a pt after primary RN had 2 previous unsuccessful attempts. After looking at pt arms for access and not seeing any good access points [REDACTED] offered to start ultrasound IV. I went to clear debris from bed picking up a used IV catheter used by previous RN not realizing the safety mechanism had not been engaged. The soiled catheter stuck my R index finger drawing blood. I washed my hands immediately after with soap and water.
