What happened
Employee was descending the middle of the stairs with a handheld toolbox in one hand. She was using the left handrail but only had her hand placed on the handrail not holding it. The employee missed the fourth step up from the middle landing and fell into the right wall with her right shoulder and landed on the step with her right knee. The employee had pain and soreness to the right knee with an abrasion that was slightly bleeding.
