EE was at floor level positioned in front of the forming tube cart in preparation for pulling the inner shell out of the tube.
What happened
EE was lifting out the inner shell of the forming tube during teardown when they lost grip of the shell. The force of the shell caused the thumb to be struck between the inner shell rim and the edge of the outer forming tube causing a laceration.
