What happened
On [REDACTED] the employee asked to take a few weeks off because his back was bothering him. On [REDACTED] he turned in OFLA paperwork because the doctor had told him he was going to need to have surgery. [REDACTED] we suddenly got a letter from a law office claiming that the employee had injured himself on [REDACTED] while chasing a calf and tripping. This is the first we had heard anything about it being a work related injury. So we are recording the info for the time being as an injury while workers comp does an investigation.
