What happened
Employee was using the side by side machine in the [REDACTED] lobby when suddenly the tank that holds the water came off and hit employee 's leg and some of the water spilled on his pants. The machine went left and the tank fell right which caused the employee to reach down in an awkward position while still operating the machine. The employee finally stopped the machine and tried to fix it that 's when the department director came through the doors and saw the employee and said he would go get a wrench. He tightened it and the employee continued the job thinking nothing was wrong until I felt a sharp pain in his shoulder.
