The employee was packing a crate with folding sides with customer parts.

The folding sides of the customer supplied crate is held up by tabs. To open the side of the crate and access the bottom employees pull down the sides which typically takes a bit of force to overcome the tabs holding them in place. In this instance a tab was broken and when the employee pulled the side of the crate the side came down easily and she fell backwards.
