Before the incident
Employee was running Robot. Side A of the Robot was running Mod Sleds and side B was running gates. The robot was running on side A as employee was loading parts on side B.
What happened
Curtains were not effective to prevent flash as the parts hit and curtain and preventing them from closing. In addition employee was sweating and kept taking safety glasses off to wipe face. This caused Arc flash in both eyes.
