Before the incident
EE was pushing the mechanical release button with the provided tool While pushing the tool inward to release the button he also had his finger positioned close to the tip of the tool which put his finger in between the tool and the edge of the fixture housing At that moment that he was pushing there the roll that was on the winder slightly turned by gravity which caused the shaft to slightly turn as well Since EE finger was between the tool and the fixture housing as noted above the slight turning motion led to a shearing action which sliced the very end tip of his left index finger
