Teammate was tightening the front right bolt on 16N forge press using a hydraulic ratchet. One of the maintenance teammates was placing the ratchet on the bolt head while the other teammate controlled the hydraulics and monitored the pressure gage. When the teammate placed the rachet on the bolt head they eft their hand on the hydraulic line to the ratchet. While the ratchet was in use the braided hose failed causing a pinhole sized puncture in the line allowing pressurized hydraulic out.
