Before the incident
Employee was working with another operator to insert the jet head and hose into the 18 diameter pipe to start high pressure water jetting the pipe.
What happened
As the operator was turning on the water and bringing the pressure up the jet hose relaxed and fell out of the pipe. Subsequently the jet head and hose propelled in the direction of [REDACTED] before he could move out of the way or the operator was able to turn off the water to stop the jet nozzle.
