On [REDACTED] at approximately [REDACTED] US FOODS Selector was selecting in location D2615801 when the incident occurred The employee stated they went to go select a package of [REDACTED] once they finished with selecting that item they started to walk their jack to slot [REDACTED] to select another item When the selector was on the way to select another item they were walking in front of the jack and had it a bit too close to them this is when they tried to come to a stop but their jack hit the back of the selectors left Achilles Once the selector injured themselves they tried to walk it off and endure the pain but they stated that the pain was too much to work through this is when they came to find a member of management to report the incident
