On [REDACTED] at approximately [REDACTED] US FOODS lift operator was working in location [REDACTED] when the incident occurred The employee stated they were cutting wrap from a pallet they had just dropped and were walking around the right side of the lift when they tripped over the metal outrigger near the floor they extended their left forearm and caught themselves on the corner of the right mast of the forklift and injured their forearm The incident did not involve other employees The injury was due to a tripping hazard Contributing factors included tripping hazard eyes not on path The employee selfadministered first aid and stated they would finish out the shift then go get the wound treated at an emergency room The employee is able to return to work
