The Selector notated that as they were finishing their batch in the cooler as they were coming down aisle 24 their jack broke rendering it completely inoperable with maintenance needing to get involved Associate [REDACTED] was in the area as the jack broke down and left to notify maintenance of the jack location and condition As they returned they saw [REDACTED] standing in front of the jack he stepped far enough out of the way for maintenance to get their jack in front of his to possibly lift and drag it out of the area As this was occurring [REDACTED] entered the aisle in the opposite direction of traffic setting himself up to push the jack out with his forks To clear the way [REDACTED] began to cross to the other side of the aisle As this was transpiring [REDACTED] began to turn his jack nipping the pole which ended up causing his jack to slide up onto the foot of [REDACTED] as he proceeded to step over the forks to get out of the way This is when the injury occurred with management
