On [REDACTED] at approximately [REDACTED] [REDACTED] delivery driver was completing a delivery Cannatas [REDACTED] Just before the injury occurred [REDACTED] was on the back of his truck stacking and scanning product When he reached down to lift a case of shortening which was within his power zone he immediately experienced a sharp rising pain through his left lower back Believing the pain was acute he attempted to continue lifting product but soon had to stop due to the worsening of the pain [REDACTED] then called his supervisor [REDACTED] and requested to be relieved of duty and seek medical attention [REDACTED] was directed to Concentra [REDACTED] located at [REDACTED]
