What happened
On [REDACTED] at approximately [REDACTED] US FOODS loader was picking up a full staged pallet at [REDACTED] when the incident occurred The employee stated they were picking up a pallet and while driving off the [REDACTED] handle turned towards rail causing their foot to get smashed between the pole and PIT The incident did not involve other employees Contributing factors include congested work area and improper driving techniques The employee was offered first aid and clinic evaluation which they accepted at this time The employee is not able to return to work as of [REDACTED] Root cause is currently being determined pending investigation
