[REDACTED] was using the electric pallet jack to put a pallet of tile into the storage unit. He accidentally broke a pallet and pieces of wood were lodged in the pallet jack preventing it from moving.

[REDACTED] attempted to dislodge the wood with his foot and move the broken pieces out of the way. He went to stomp down a piece of wood that was sticking up and the piece penetrated the bottom of his work boot and pierced his right foot.

Contusion of foot including toes right. Foreign body of skin of plantar aspect of right foot.
