Inspecting mezzanine area that he had to access using his extension ladder since the new construction did not have fixed ladder installed.
What happened
Employee was in a dimly lit room when he went to disengage extension ladder so it would collapse down. In doing so he had grip on the ladder and stepped backwards into an unmarked hole on the construction site that caused him to fall a few feet into the hole with his ladder.
