What happened
EE worked extra hours working a 12 hour shift [REDACTED]. EE was operating the bulk melter from [REDACTED]-[REDACTED]. EE stated he did not notice any pain strain or Un comfortability while at work or during any work task. EE noticed slight pain before going to bed didn’t think anything of it and woke up [REDACTED] and couldn 't lift his left arm above shoulder height. EE doesn’t recall any moment of pain or strain during the shift just that he was loading the bulk melter.
