Before the incident
Manpower employee explained to me that as she was making the turn she inadvertently put the wrong input into the controls of the forklift causing the forklift to speed up. As the forklift sped up she stated the movement of the forklift caused her left foot to come out of the operator space of the forklift. From there the forklift continued to travel backwards into the turn she was making trapping her foot between forklift and a pallet jack sitting next to compactor.
