Operator was tightening the base bolts that secure the Vent port to the [REDACTED] extruder after they had just re installed the Vent port during a [REDACTED] event. While doing so the operator lost his footing and fell to the ground due to the floor being a wet surface from the ice melting away on the barrel cooling lines and reaching the floor. As the employee fell they struck the back of their head on a barrel insulation cover that was on the floor waiting to be re installed after the Vent port bolt tightening was complete causing a laceration to the back of the head.
