HE WAS PULLING A WHEEL OFF THE TOP SHELF IN THE WAREHOUSE. EMPLOYEES ARE REQUIRED TO USE A LADDER WHEN PULLING OFF TOP SHELF. EMPLOYEE USED LADDER HOWEVER HE DID NOT OPEN IT UP COMPLETELY AS TRAINED AND INSTEAD LEANED THE LADDER AGAINST WAREHOUSE RACKING AND CLIMBED UP. THE ITEM BEING PULLED THEN SHIFTED IN THE BOX AND HE FELT A STRAIN IN HIS BACK AS HE TRIED TO KEEP HIS BALANACE
