Before the incident
The employee was working as part of a 4 man team attached to the Racking Crew putting up the [REDACTED] Changer tubes onto the piles.
What happened
One of the workers of the four man team slipped on a root covered in mud. This motion caused that worker to suddenly lower his end of the tube. The tube was approximately 30 feet long. This motion also caused the opposite end of the tube to suddenly raise in a upward motion. The sudden upwards motion on the opposite end caused the tube to strike the effected employee in the mouth on the left side of his upper lip which caused a laceration.
