EE was backing out of the building into the shuttle pad with a double stack of shingles on their forklift.
What happened
While the EE was backing out and shifting into forward they were bumped by a flatbed shipping truck that was backing into the normal loading space. Neither of the drivers saw each other. EE was wearing all required PPE including seatbelt.
