Before the incident
Employee was assisting with laying a motor and fan down on a pallet to send out for repair. Contractor was on forklift using attachement to move the motor. Employee was on floor guiding motor. Believe motor slid off of fork an attachment hit employee on left hand.

When attachment slid off of forklift motor dropped to the ground and attachment fell hitting employee 's left hand before hitting the ground
