THE EMPLOYEE WAS ASSEMBLING THE LEFT COUNTERBALANCE ON AN ERFURT TRIM PRESS.

The employee and an overhead crane operator were working on assembling the counterbalance on the trim press. The cylinder was being lifted with a web strap and overhead crane the web strap was feed through an opening in the press and was pulling up the cylinder through the cavity. The injured employee was using a crowbar to align the cylinder with the pinhole on the press. He had placed the crowbar inside the pinhole on the cylinder and was pushing left and right to make the alignment. The web strap caught itself on an angle iron insidethe crane operator was unable to see this and continued lifting on the strap the strap broke causing the 100lb cylinder to fall back down swinging the crowbar forward. This caused the employee to hit his left on against electrical conduit box and the crowbar.
