Nursing Assessment: Spoke with EE regarding injury report. A patient lunged at a sitter 's neck. The EE redirected the patient by shoulders. Patient swung fist at nurse. Hit sitter with elbow and punched EE in the right eye. At this time the eye is all colors of bruising with slight swelling on lower eyelid but not painful per EE 's statement. EE went to ED per One Call instruction. EE is scheduled for follow up with Dr. [REDACTED] on [REDACTED] [REDACTED] at [REDACTED] for a return to work note. EE declined being seen in Occ Health at this time. EE verbally acknowledged receiving WC booklet information via email; that WM is self-insured and OH directs all care for the injury. EE verbally acknowledged appointment date time and location of f u with Dr. [REDACTED]. EE verbally acknowledged the need to bring all ED paperwork to the appointment with Dr. [REDACTED].
