What happened
On [REDACTED] at approximately [REDACTED] [REDACTED] was assigned to the Zim Atlantic Gang 1 lashers. While conducting his assigned work on [REDACTED] Mr. [REDACTED] stepped onto the access grate covering the hatch for below deck access. When Mr. [REDACTED] stepped onto the grate it gave way and he fell through to the hatch cover below. The fall was less than 3 feet. When he fell Mr. [REDACTED] scraped his right shin on the lashing bridge. Mr. [REDACTED] tried to catch himself by bracing with his left hand which jammed his shoulder and caused pain in his shoulder and left side of his upper back. Mr. [REDACTED] decided to Self- Transport to Urgent Care of [REDACTED]. Attached below is a picture of his shin with that injury shown.
