Before the incident
Employee was helping another employee re-rack an axle. The axle was on the forks and they were attempting to lift the axle off with a hoist. The hoist was side loaded and the wrong lifting device was being used.
What happened
When axle was lifted the device slid along the axle as it tried to center itself under the hoist it caught the employee 's hand between it and the end of the axle.
