Team member was assembling a fly into the boom for [REDACTED] As he was lining up the fly it was misaligned and had some slight tension When they let off the crane controller button the fly backed out of the boom The operator grabbed the top of the fly Then the fly went back into the boom taking the team members hand with it Team Members hand was crushed in between the two
