Employee was Installing a Large black pin into a winch assembly To install the winch you must swing a 4lbs hammer roughly 15 times before the pin goes through the 3 holes When installing the pin the back broke and they needed to uninstall the pin to replace it with a conforming pin When doing that the pin flew out The winch fell on his foot and the winch grazed his left pointer finger
