Around [REDACTED] patient bed alarm went off followed by tele monitor going off when myself and nurse lovely arrived to the room the patient was already sitting on the side of the bed.. Informed patient that she is supposed to call if she needs to go to restroom like she has several times throughout the night patient stated she was going to call. I proceeded to standby assist while the patients walks to the restroom once in the restroom I begin to assist patient pull down her pull-up to prevent her from pulling on her drains. Patient lost her balance and begin to fall forwards. I tried grabbing the patient 's waist but could not hold patient fell to her side hitting her left shoulder on the base of the shower.
