What happened
Forklift operator at stop was taking long crate off the back of drivers truck. The crate started to tilt so the driver grabbed the crate to stop from failing over to balance. The forklift operator then tried to push the crate back on the truck slightly and pushed forward however the crate did not move and the drivers hand was still there trying to balance crate and was pinched between crate and metal on forklift.
