What happened
Employee had lifted with forklift a trash bin (heavy) and tilted the forks in order to empty trash to the main open dumpster. It appeared that the Bin was going to fall inside the Dumpster. Employee was off the forklift and reached to hold on to the bin and prevent it from falling in the Dumpster. While doing so holding the bin with the right hand and steadying himself with the left hand holding on the the edge on the open dumpster the Lid of the bin came down closing as he was pulling the bin
