Employee was re-entering building through exterior door following a break.

When the employee attempted to enter the building the door quickly closed behind them before they could fully cross the threshold and the door struck the employee from behind. The employee lost their footing and fell.

Employee struck their back on the floor and the door struck their head as it was closing. Employee felt pain on the left side of their neck in their lower back & left side and felt a tingling sensation in their left leg.
