[REDACTED] was sitting at dining room table feeding client. Offered him one of each items she had on the plate he refused. Then he started hitting pushing pinching. He pinched her on her arm. Other staff got on other side of him. Client got up got ahold of her arm again and when she moved it ripped in a triangle tear. Right elbow skin tear and bruise on forearm.
