Before the incident
The contingent team member was in the process of moving a [REDACTED] (pressure vessel) from the kit cart to the prep table.
What happened
The tank was flange-side down with the lifting lugs facing up as they should have been. Instead of attaching the rigging to the lifting lugs he attached the lifting fixture to the endplates. By doing this the load was extremely top heavy. The operator positioned one hand on the lifting fixture chain before the lift .When the operator went to lift the tank and swing it out laterally to the work table the tank flipped forward 180 degrees pinching the operators hand between the steel chain and the tank.
