Before the incident
Maintenance installed a inline filter on the NSM tank. Both the operator and maintenance started to blow back the line for 20mins as they should have. Operator then said he turned off the [REDACTED] and opened the bottom valve of the tank to let the capro monomer circulate. Operator said the agitator was on and the pump was spinning perfectly fine for 10 mins.
What happened
As operator was getting ready to begin casting he felt a burning sensation on his back when he turned completely around the monomer was already spraying at him. The operator had monomer on his back and got splashed on his left eye.
