EE was using the forklift Toyota [REDACTED] to remove a stack of pallets to the pallet truck when one of the pallets slid and cut him on the top of his head EE was taken to Concentra Immediate Care Center where he received 5 staples in the top of his head Follow up appointment is [REDACTED] The investigation found there were slip sheets between the pallets which became lodged between the mast and the frame of the forklift
