What happened
TM was repairing pallets. TM was about to remove the t-1 board when the forklift driver was coming to the tipper to put another stack in the infeed. The top half of the stack slipped. The driver pressed on the brakes and caused the pallets to fall. TM was struck by them on the upper back. TMs left arm was also pinned to the table.
