On [REDACTED] a shop mechanic was disassembling a large hydraulic cylinder for repairs. The employee was attempting to remove the cylinder connecting block from the threaded rod end. They applied upward force on a sleever bar (pry bar) through the hole in the block to unscrew the block while holding the rod steady with their right hand and using a pair of [REDACTED] pliers in their left hand. When the connecting block “broke loose” suddenly and began to turn the employee lost control of the sleever bar which caused the flat end to strike their chin.
