Before the incident
Material picker employee was standing on the backside of single rack between dock 10 11. A forklift operator was removing a pallet from the other side of this rack. When the operator raised the pallet the leading tips of the forks lifted up the pallet stopper bar on the backside of the rack. The spring activated safety pins on the pallet stoppers disengaged resulting in the pallet stopper falling down from the rack striking the head of the employee that was standing next to the rack.
