What happened
EE was using a Jungheinrich electric pallet jack to move a pallet of ship out drums that had been placed on a broken pallet to the hoist area so drums could be transferred to a new pallet. As EE angled the pallet toward the hoist it clipped a pallet by the machine causing a drum to fall off to the right side of the pallet that EE was moving. The pallet jack continued forward causing EE left foot to get trapped underneath the pallet jack.
