What happened
Employee was driving a forklift with the mixing bucket on the forks. The employee was on a forklift he does not normally use and had his forks elevated. When entering the maintenance shop the forklift scraped the bottom of the doorway. The operator stopped and looked up and dirt debris feel onto the employee. The employee was wearing his safety glasses but the debris fell through the top of the glasses. The employee immediately lowered the forklift forks and shut the forklift off. He went to the eye wash station and rinsed his eyes. He than used the salin eye drops out of the first aid. Went to production manager to report the incident. Plant Manager was immediately notified.
