What happened
Employee was descending an approximately 6.5’ tall fixed ladder on separator 2700. The EE lost their footing and caught themselves with their hands. They were holding the bottom of the gate with their left hand and had tools in their right hand when they slipped. The right-hand grip failed causing their entire body weight to be supported by their left hand and arm until they could regain their footing
