What happened

At [REDACTED] I received a call from our mechanic [REDACTED] reporting that an incident had just occurred. [REDACTED] stated [REDACTED] one of two certified install dismantle technicians had just been struck by a piece of the [REDACTED]. I arrived on scene at approximately [REDACTED] in which EMS had just arrived and were evaluating [REDACTED]. The crew working onsite consisted of Equipment Operator [REDACTED] and Certified Scaffold Technicians [REDACTED] and [REDACTED]. [REDACTED] our lead mechanic was an observer onsite at the time of the incident. Re-Create Companies Superintendent [REDACTED] and McShane Construction Project Superintendent [REDACTED] were also present upon my arrival but neither witnessed the incident. According to the Equipment Operator while setting the final bridge for the hydraulic scaffold he was in the process of pushing forward when the bridge became stuck.
