What happened

while at work was walking and putting weight on my left foot and felt a sharp pain - Ms. [REDACTED] states that she had injured her left foot when she was walking from one residents room to another. She had felt a snap in her left foot. She did not run twist her foot the ground was even and she did not step on anything. She reported the injury [REDACTED] when she realized the pain was not going away. She was seen at Concentra on [REDACTED] and the doctor states that her injury is not related to her work and she will need to follow up with her PCP. We did obtain a legal opinion and there basis for denial as there is a lack of medical causation in the medical reports.
