What happened
Employee was removing a bolt and nut using a hand held Allen key in the right hand and an impact drill in the left hand. While attempting to loosen the nut the right hand holding the Allen key slipped and the right thumb made contact with the spinning bolt head causing a 3cm vertical laceration on the right thumb.
