EE reports pain in his right shoulder that radiates. EE states that this injury occurred on [REDACTED] but is unsure of the mechanism of injury the activity he was engaged in or the exact time of the injury. EE reported non-work related shoulder pain to his manager on [REDACTED] and asked for additional time off work for the holiday weekend. EE visited his personal physician during time off and was prescribed anti-inflammatory and steroids. On [REDACTED] upon return from the weekend EE now states that this is a work-related injury and requests medical care. No witnesses.
